Confirmations
A confirmation will be sent via email to all registrants who register at least one week in advance.

Cancellation Policy
A $25 non-refundable processing fee will be retained for each cancellation.  Cancellations must be made in writing no later than February 3, 2012, and emailed to
info@CoalitionCCC.org
or faxed to (888) 789-9475.  No refunds will be made after this date.  Substitutions are encouraged.  Space is limited.

Hotel Information
The Catamaran Resort Hotel and Spa, 3999 Mission Blvd, San Diego is offering a group rate of $139/night, plus tax, single or double occupancy for a limited number of rooms.  The deadline for reserving rooms at this rate is January 17, 2012.  After that date, reservations will be confirmated subject to availability.  Contact the hotel at (858) 488-1081 and ask for the CCCC/POLST group rate.

Education Credit Information
Continuing Education credit will be provided for the CCCC Annual Conference on February 15 and the POLST National Conference on February 16.  Full attendance of the education sessions in their entirity is required to receive professional continuing education credit.  Attendees must sign in at the conference and include their professional license number.  Certificates of attendance will be available at the end of the conference.  The Riverside County Medical Association (RCMA) has graciously offered to provide CME credits.
  • Nursing
    Provider approved by the California Board of Registered Nursing, Provider Number CEP 15403 for 5 contact hours for each day.
  • Social Work
    This course meets the qualifications for 5 hours of continuing education credit for each day for MFTs and LCSWs as required by the California Board of Behavioral Sciences, Provider PCE 4576.
  • Physicians
    The Riverside County Medical Association is accredited by the Institute for Medical Quality/California Medical Association to provide continuing medical education for physicians.  The RCMA takes responsibility for the content, quality and scientific integrity of this CME activity.  This conference has been submitted to the RCMA for 5 AMA PRA Category 1 Credits for each day.
  • Nursing Home Administrator
    These courses have been submitted to the California Nursing Home Administrator Program for 5 NHAP/P Category 1 Credits for each day.  CCCC is an approved NHAP provider.
